Consent to Telehealth

This Consent to Telehealth describes how care is delivered through Stateside Scripts, Inc. and the things you should understand before receiving telehealth services. By proceeding, you confirm that you have read and understood this consent and agree to receive care via telehealth.

01What telehealth is

Telehealth is the delivery of healthcare using technology — such as secure messaging, questionnaires, photos, phone, or video — when you and your provider are in different locations. Through Stateside, you connect with independent, licensed providers who review your information and determine whether treatment is appropriate.

02Potential benefits

Telehealth can make care more convenient and accessible, reduce travel, and allow timely communication with a provider. It may also let a provider deliver care efficiently when an in-person visit is not necessary.

03Potential risks and limitations

  • Without an in-person physical exam, a provider may have less information than at an in-office visit, which can affect diagnosis and treatment.
  • Technology can fail or be delayed, and electronic transmissions could, in rare cases, be disrupted or accessed by unauthorized people despite safeguards.
  • In some cases a provider may determine that telehealth is not appropriate for you and may recommend in-person care.
  • As with any care, treatment may not produce the desired result, and there are risks of side effects or complications.

04Not for emergencies

Telehealth is not for emergencies. If you have a medical emergency, call 911 or go to the nearest emergency room. Stateside providers cannot provide emergency services.

05Alternatives

You have the right to seek in-person care instead of telehealth at any time. Your provider can discuss alternatives to any recommended treatment, including their risks and benefits.

06Medical records and privacy

Information shared during telehealth is documented and protected consistent with applicable law and our Privacy Policy. You may request a copy of your records as described there or by contacting your provider.

07Electronic communications

By using the services, you consent to receive communications electronically, including secure messages and emails related to your care, account, and orders, as permitted by law.

08Your rights and consent

Your participation is voluntary, and you may withhold or withdraw consent and stop telehealth services at any time without affecting your right to future care. Withdrawing consent does not apply retroactively to care already provided. You understand that no specific outcome is guaranteed.

10Acknowledgment

By proceeding, you acknowledge that you have read this consent, had the opportunity to ask questions, and agree to receive care through telehealth as described above.
